Personally Identifiable Information such as SSN, driver license number, or birthdate must be redacted from all documents submitted. A completed copy of the Universal Rebate Form (v. 04/25) signed by the customer and the Dealer to show the rebate offer was presented to the customer and how the customer chose to apply the rebate(s). completed copy of the Retail Buyers Order and/or Financing Contract of the Eligible Vehicle, signed by the customer and the Dealer disclosing the applicable Lease-End Loyalty Rebate amount as a separate line item and how it was applied to the sale transaction (if applicable).
Military Customer Rebate*

*Eligible Customers (or spouse / surviving spouse) must be active duty or active reserve personnel, or veterans who served in the U.S. Military, and U.S. Military refers exclusively to United States Army, Navy, Air Force, Space Force, Marine Corps, National Guard, Reserve, and Coast Guard, who were discharged or released therefrom under conditions other than dishonorable, at the commencement of the Program Period. Proof of U.S. Military status required. Proof of marital status (e.g. marriage certificate) required if only spouse is listed on the purchase/lease agreement.
